Our San Ramon-based team is growing, and were seeking a dedicated Occupational Therapist (OT) to help clients regain independence, improve daily living skills, and achieve their highest possible quality of life. In this role, youll design and implement individualized therapy plans, work closely with caregivers and clinical staff, and ensure each client receives seamless, personalized care tailored to their unique needs. Position Summary We are seeking a skilled and compassionate Occupational Therapist (OT) to join our home health team. In this role, you will assess patient needs, design individualized care plans, and deliver therapeutic interventions that improve daily living skills and safety in the home. Key Responsibilities Perform initial evaluations to assess ADL/IADL capabilities and home safety risks Develop goal-oriented treatment plans to enhance independence in self-care and daily tasks Address physical, sensory, emotional, and cognitive barriers to function Educate patients and caregivers on adaptive techniques and assistive device use Recommend home modifications to reduce risks and improve accessibility Document care accurately in the EMR (WellSky) and maintain compliance with regulations Collaborate with PTs, RNs, and interdisciplinary team members for coordinated care Participate in team meetings, QA initiatives, and case conferences Qualifications Active, unrestricted California Occupational Therapist (OT) license Minimum 1 year of clinical experience (home health or geriatrics preferred) Current CPR certification Reliable transportation and valid CA drivers license EMR documentation experience preferred Strong communication, adaptability, and problem-solving skills Compensation $55 - $70 per hour Benefits (As Eligible) Flexible scheduling for OTs seeking work-life balance Access to clinical and administrative support from Case Managers and office staff Participation in recognition programs (e.g., Clinician of the Week and Clinician of the Month) Referral bonuses for bringing in other licensed clinicians Paid Time Off (PTO) available to Per Diem staff after completing 720 hours of service Why Join Qualicare?
